Mopane Community Health
I just wanted to acknowledge the incredible work Shanaaz Nisha (NDIS Support Coordinator) has done to assist with a shared NDIS participants discharge from hospital into a new home. Shanaaz has been professional, thorough, compassionate, knowledgeable, efficient, reliable and just an all-round incredible team member and resource. Although it was a whole team effort to get the participant into a new home after an incredibly long hospital stay, I don't think we would have had such great success if it wasn't for Shanaaz's experience and hard work. Thank you!
